Step 1 — Read & save the Consent

Step 2 — Have these ready for your provider

• Signed Consent (printed or ready to e-sign on your phone/tablet)

• Online Intake completed (owner & pet details, meds/allergies, behavior notes, rabies date/clinic)

• Rabies certificate (photo or printout is fine)

• Medication/allergy list (name + dose)

• Safe workspace: small, well-lit area; access to an outlet and sink helps

• Towels and a few treats your pet likes

Day-of prep

• Have your pet leashed or in a carrier when we arrive

• Give a quick potty break beforehand

• Skip very hard chews the day of your visit

• Please separate other pets during the appointment

• If your pet has a muzzle, have it handy (we use low-stress handling)

• Parking/access info: let us know gate codes, stairs, or special parking notes

After your visit
